We're here to help

Choosing a new GP is an important decision. At Terrey Hills Family Surgery, we aim to make your first visit as easy and comfortable as possible. From booking your appointment to transferring your medical history, our friendly team is here to help every step of the way.

Before Your Appointment

To help us prepare for your visit, we recommend:

  • Complete your new patient registration form online (this will be sent to your email) before your appointment. 
  • Bring your Medicare card and any concession or DVA cards.
  • Bring a list of your current medications.
  • Bring any recent test results, referrals or specialist letters if relevant.

Your first Visit

This is an opportunity for your GP to get to know you and understand your health history. Your doctor may discuss:

  • Your medical history
  • Current medications
  • Existing health conditions
  • Preventive health & screening
  • Your healthcare goals

If you have any concerns, let us know when booking so we can arrange a longer appointment.  

Medical records

If you're changing from another medical practice, we can help make the transition easy.

With your consent, we can request a copy of your medical records from your previous GP to help ensure continuity of your care.
