Living with a chronic health condition can be complex, but you do not have to manage it alone. At Terrey Hills Family Surgery, our GPs provide coordinated, long-term care in line with Medicare Chronic Disease Management (CDM) arrangements to support your ongoing health and wellbeing.

Whether you have recently been diagnosed or are living with a long-term condition, we work with you to develop a personalised care plan focused on improving quality of life, preventing complications, and supporting your independence.

Common Chronic Conditions Management in General Practice

Patients with chronic or complex conditions are identified through routine consultations, preventive health assessments, and clinical recall systems. Chronic conditions may include, but are not limited to:

Medicare Chronic Disease Management Services

Our practice provides structured care in accordance with Medicare Benefits Schedule (MBS) requirements, which may include:

  • GP Chronic Condition Management Plans (GPCCMP)
  • Regular GP reviews and monitoring
  • Medication review and optimisation
  •  Referrals to eligible allied health services under Medicare, where clinically appropriate
  • Preventive care, screening, and risk factor management
  • Support for lifestyle modification and self-management

Coordinated, Ongoing Care

Our GPs work collaboratively with you and, where appropriate, your carers and other healthcare providers to ensure your care is coordinated, continuous, and responsive to your needs.

We support shared decision-making and provide education and guidance to enable you to actively participate in managing your health.

Through structured chronic disease care, we aim to improve health outcomes, reduce complications, and support long-term wellbeing in line with best-practice general practice care.

Clinical Governance and Quality Improvement

Chronic disease management processes are subject to ongoing quality improvement activities, including clinical audit, guideline review, and adherence to evidence-based practice. Systems are in place to support safe, consistent, and effective chronic disease care in accordance with RACGP Standards for General Practice.
